About our Host

The Forum at Memorial is a full-service senior living community providing elegant independent living, assisted living apartments and Healthcare Center for skilled nursing and rehabilitation. Also provided is Five Star’s award-winning Bridge to Rediscovery Alzheimer’s and Dementia Care program, as well as guest and respite/short stay programs.

Cost Dinner & Program

The Dinner/Program cost is $25 for Non-Members and $15 for Supporting Members. For Students/Ombudsmen who are Members, the evening fee is $5. Dinner is free for members who are at the Sustaining & Corporate Membership Levels. The reduced fee for Guests attending with a Member is $20.

The University of Texas Houston Health Science Center at Houston School of Nursing is an approved provider of continuing nursing education by the Texas Nurses Association, an accredited approver by the American Nurses Credentialing Center’s Commission on Accreditation. This activity provides 1.0 contact hours of nursing continuing education and social work education.
